PVAMC Geriatrics And Extended Care

Program Description

Geriatrics and Extended Care provides services for Veterans who are elderly and have complex needs, and Veterans of any age who need daily support and assistance. Veterans can receive care at home, at VA medical centers or in the community.

Services

  • In-Home and Community-based Services: Adult Day Health Care, Home-based Primary Care, Homemaker and Home Health Aide Services.
  • Hospice and Palliative Care.
  • Respite Care.
  • Nursing Home Services: The VA contracts with Community Nursing Homes to provide nursing home care to veterans.

Eligibility

  • In-Home and Community-based Services: All enrolled veterans are eligible, but the request for services will come from the primary care physician. (Social work contact is Max Arias, 401-273-7100 x4282.)
  • Hospice and Palliative Care: The screening for this service is through the palliative care team. No service-connected disability is required. (Social work contact is Sue Cesaro.)
  • Nursing Home Services: 70+% service-connected disability. (Social work contact is Juleigh Wilson, 401-273-7100 x6703.)
